Event Overview
This two-part series will cover the following during Session 2.
Session 2
This advanced training takes a deeper dive into effective Huddle practices with an applied focus on improving colorectal cancer screening using the Azara platform. Participants will explore real-life implementation strategies, engage with tools and data, and learn tactics to sustain and scale their Huddle practice.
Session 2 Learning Objectives:
- Implement best practices for effective Huddles, including agenda structure, frequency, facilitation, and tracking outcomes.
- Use the Azara platform to identify patients due for colorectal cancer screening and incorporate this information into daily or weekly Huddles.
- Apply a Huddle workflow that targets colorectal cancer screening gaps, utilizing team-based care strategies (e.g., pre-visit planning, outreach roles).
- Evaluate their current Huddle practice and identify opportunities for improvement using structured tools or checklists.
- Develop a plan to sustain and evolve the Huddle model within their team or organization, including leadership engagement, training new staff, and regular feedback loops.
