This session introduces the concept of Huddles as a powerful communication tool within healthcare organizations. Attendees will explore the why behind Huddles, different types (administrative vs. care team), and strategies for successfully adopting Huddles into everyday practice to build a culture of proactive care and collaboration.

Session 1 Learning Objectives:

  • Define what a Huddle is and articulate its role in improving team communication, patient care, and operational efficiency.
  • Differentiate between administrative and care team Huddles, including their goals, participants, frequency, and topics.
  • Describe the benefits of Huddles for healthcare teams and patients, with a focus on improving outcomes and addressing gaps in care.
  • Identify team roles and responsibilities within various types of Huddles.
  • Recognize key factors for integrating Huddles into an organization’s culture, including leadership support, scheduling, accountability, and documentation.

This advanced training takes a deeper dive into effective Huddle practices with an applied focus on improving colorectal cancer screening using the Azara platform. Participants will explore real-life implementation strategies, engage with tools and data, and learn tactics to sustain and scale their Huddle practice.

Session 2 Learning Objectives:

  • Implement best practices for effective Huddles, including agenda structure, frequency, facilitation, and tracking outcomes.
  • Use the Azara platform to identify patients due for colorectal cancer screening and incorporate this information into daily or weekly Huddles.
  • Apply a Huddle workflow that targets colorectal cancer screening gaps, utilizing team-based care strategies (e.g., pre-visit planning, outreach roles).
  • Evaluate their current Huddle practice and identify opportunities for improvement using structured tools or checklists.
  • Develop a plan to sustain and evolve the Huddle model within their team or organization, including leadership engagement, training new staff, and regular feedback loops.
