Elevating Depression Care Through Measurement-Based Practice is a three-part virtual training series designed to strengthen clinical skills, enhance system-wide collaboration, and support long-term sustainability in depression care across the primary care setting. Each session builds on the previous one, guiding participants from foundational concepts to practical application and systems-level strategy. Across the series, participants will deepen their understanding of measurement-based care (MBC), learn how to operationalize the Collaborative Care Management Model (CoCM), and explore approaches for sustaining effective mental health services over time. Together, these sessions equip care managers, administrators, medical and behavioral health providers with actionable tools to improve patient outcomes and meet quality standards such as Uniform Data System (UDS) depression measures. 

*This series is offered jointly by the Center for Mental Health Implementation Support (CMHIS) and the Association for Utah Community Health (AUCH). CMHIS will lead Sessions 1 and 3, focusing on measurement-based care and sustainability, while AUCH will lead Session 2, highlighting the Collaborative Care Management Model (CoCM) and its application in primary care.

February 19, 2026 | 10-11am MST | Virtual

  • This session introduces measurement-based care (MBC), an approach that uses regularly collected patient-reported outcome measures to assess progress and inform clinical decision-making. Participants will learn how MBC creates real-time opportunities for communication, supports timely treatment adjustments, and enhances treatment outcomes. Despite its strong evidence base, MBC remains underutilized—this session offers practical strategies to integrate it into everyday practice. 

February 26, 2026 | 10-11am MST | Virtual

  • Building on the foundation of MBC, this session explores the Collaborative Care Management Model (CoCM), an evidence-based approach that brings together primary care providers, behavioral health care managers, and psychiatric consultants. Participants will learn how to incorporate outcome data into treatment planning, meet UDS depression screening and remission requirements, and strengthen team communication and care coordination. Concrete examples and workflow strategies will support participants in implementing CoCM effectively in their own settings. 

March 5, 2026 | 10-11am MST | Virtual

  • The final session focuses on ensuring long-term sustainability of depression care services. Participants will examine key components of sustainability and sustainment—including funding, staffing, partnerships, and organizational readiness—and learn strategies to maintain effective programs over time. The session highlights how to embed evidence-based practices into organizational systems so that positive patient outcomes can be consistently achieved and supported well into the future.
