As rates of homelessness and conditions of unstable housing increase with our community, health care teams in all settings are tasked to support patient success with chronic disease management despite these circumstances. Join AUCH, the National Health Care for the Homeless Council, and local experts for an informative session on best practices.

Objectives include:

  • Learn practical and feasible approaches to Diabetes management for persons experiencing homelessness and unstable housing
  • Review recommendations from nationally developed resources that provide culturally tailored guidance on Diabetes self-management and diet for our patients
  • Learn from local Utah providers that serve affected populations

Alaina Boyer, PhD, Senior Director of Programs, National Health Care for the Homeless Council

Dr. Boyer has been with The National Health Care for the Homeless Council for 8 years and is currently the Senior Director of Programs at the Council. She oversees the Directors of Implementation Research, Clinical and Quality Improvement, and Community Engagement. Dr. Boyer is also responsible for managing the Health Resource and Services Administration National Training and Technical Assistance Cooperative Agreement as well as other federal and non-federal grants. Dr. Boyer leads and manages peer to peer learning collaboratives and presents national webinars on content related to the intersection of health and homelessness, social drivers of health, and race and equity and homelessness to the Health Care for the Homeless health centers, all HRSA supported health centers, and the homelessness community at large.

Caitlin Synovec, OTD, OTR/L, BCMH, Assistant Director for Medical Respite, National Health Care for the Homeless Council

Caitlin Synovec, OTD, OTR/L, BCMH is an occupational therapist with clinical experience in working with adults experiencing homelessness to improve quality of life and engagement in their preferred communities. She has completed clinical research to address the intersection of homelessness, poverty, brain injury, behavioral health, and chronic conditions, and their impact on health and community living. Caitlin’s experience also includes program development for collaborative and interdisciplinary care, as well as clinical education and training for health care and direct service providers. She is currently the Assistant Director of Medical Respite Care for the National Health Care for the Homeless Council, providing training, technical assistance, and advocacy to advance the field of medical respite and homeless health care.

Van Aston, PA, Medical Director, Fourth Street Clinic

Van Aston – Van began working with people experiencing homelessness in 2015 at the Hope Clinic, located inside the Lantern House Shelter in Ogden. Together with the clinic director, he worked to build the clinic and its community impact. In 2022, Van joined the team at Fourth Street Clinic as the Medical Outreach Street Team (MOST) provider. In October 2023, he was named Medical Director.

Karmin Bell, APRN, Associate Medical Director, Mountainlands Family Health Center

Karmin Bell, APRN is the Deputy Medical Director at Mountainlands Community Health Center. With 15 years of experience at Mountainlands, Karmin has dedicated the past eight years to serving patients experiencing homelessness at the East Bay site. Passionate about providing compassionate, high-quality care, she plays a vital role in ensuring accessible healthcare for vulnerable populations.

Tanner Davidson MSN-RN, Mountainlands Family Health Center

Tanner is a nurse at Mountainlands Family Health Center where he works with patients from underserved communities including the homeless. He works with these patients specifically on health education including diabetes, hypertension, and transitional care management. He has been working in community healthcare since 2020. He is passionate about providing healthcare to underserved populations and have conducted several studies on the how the American healthcare system treats underserved populations such as Spanish-speakers and low-income families.
