Health Centers serving Utah are active participants in research and quality improvement projects that drive better patient care among Utah’s underserved populations. Health centers are interested in projects that make it easier to deliver quality care and the infrastructure to support that work (e.g., population health management, data, technology, staffing support, patient outreach, knowledge investment, clinical materials, lab support, etc.). Azara Healthcare
AUCH members use Azara Healthcare for population health management, and data analysis and reporting. Projects that advance the use of this platform to reduce data management and reporting burdens are valuable to health centers. Learn more about Azara here.
Care Message
A subset of Utah health centers will launch CareMessage as a patient messaging platform Q2 of 2025. Projects that leverage existing patient messaging infrastructure include a ‘set and forget’ format to reduce staffing burden are of interest to health centers. Learn more about CareMessage here.
UDS Measures for Utah Health Centers
The Uniform Data System (UDS) is a standardized data set and annual program requirement that is defined in Section 330 of the Public Health Service Act. Through the UDS, health centers report annually on patient characteristics, services provided, clinical processes and health outcomes, staffing, patients’ use of services, and costs and revenues. These data help inform efforts to further expand access to care, address health disparities, improve quality of care, and reduce health care costs. UDS data are also used to document program effectiveness; identify administrative, clinical, and financial trends over time; and compare clinical quality measures (CQMs) with national benchmarks.
