Position Title: Community Health Worker (CHW) – SCALEUP Patient Navigator

Position Location: Salt Lake County

Start Date: January 2021

End Date: This is a temporary position through August 22, 2021 with possibility for extension depending on project funding.

Organizational Background/Mission

For over 30 years, the Association for Utah Community Health (AUCH) has represented Utah's Health Centers and their patients. As the Primary Care Association in the state, we provide over 12,000 hours of training and technical assistance to 13 Health Centers and 5 affiliate members each year. Our members include Federal Health Center awardees who provide comprehensive, high-quality primary and preventive health care services to all individuals, with or without insurance, regardless of ability to pay. Health Centers are community-based, patient-directed organizations that eliminate geographical and financial barriers and serve populations with limited access to care. In support of our members and the people they serve, AUCH helps reduce barriers to health care through health promotion, community engagement and development, education, and policy analysis.

Position Description:

CHWs are trusted, knowledgeable frontline health personnel who typically come from the communities they serve. CHWs bridge cultural and linguistic barriers, expand access to coverage and care, and improve health outcomes.

AUCH is hiring CHWs to work in local communities as part of COVID-19 response efforts. CHWs will work primarily with underserved communities at risk for exposure throughout Utah by connecting individuals and families to COVID-19 testing at local health centers.

CHW responsibilities include, but may not be limited to, the following:

  • Establishing trusting relationships with patients and their families.
  • Providing education about COVID-19.
  • Helping people who are exposed to or infected with COVID-19 to access testing and/or understand what is required for quarantine and isolation.
  • Screening patients for Social Determinant of Health (SDOH) needs and connecting patients to community resources to address these needs.
  • Helping patients to establish a medical home if needed.
  • Documenting activities and outcomes in appropriate databases.
  • Attending team meetings to discuss progress, successes, and challenges.
  • Other duties as assigned.

As part of AUCH’s continued efforts to reduce the impact of the COVID-19 pandemic, most of our staff are working remotely. Should you be required to work at any community sites, you will receive support, training and personal protective equipment to best protect yourself from COVID-19 to safely work with patients and their families.

Full-Time, Non-Exempt                              CHS Project Manager                                        $15-$17 per hour

Qualifications and Requirements: The following requirements must be met to be considered for the program:

  • High school diploma or GED
  • Written and oral fluency in English and Spanish required, other languages desired
  • Reliable transportation
  • Ability to work remotely and access to a reliable internet connection
  • Basic computer skills
  • Familiarity with documenting activities and entering data
  • Ability to develop and maintain positive relationships with patients and families
  • Understanding of, and sensitivity to, targeted communities
  • Good communication skills such as listening, speaking, and writing
  • Ability and willingness to provide emotional support, encouragement, and motivation to patients
  • Ability to work independently and as part of a group
  • Willingness to take initiative and be pro-active in daily work
  • Experience working within community partnerships preferred
  • Experience working as a Community Health Worker, or in a similar capacity, for at least one year is preferred
