AUCH Quarterly Learning Collaborative: Integrated Care

Overview: Integrated care is “the care that results from a practice team of primary care and behavioral health clinicians, working together with patients and families, using a systematic and cost-effective approach to provide patient-centered care for a defined population.

Whether you are new to the Integration journey, or a seasoned pro on the Integration continuum, please join us for our 2nd Quarterly Learning Collaborative on November 9th as we continue to explore what that journey looks like for you and your team. Peer groups will also have the opportunity to connect, in-person, with their peers, and explore promising practices. 

Cost:

• Full registration, includes peer group, training session, and learning collaborative- breakfast, lunch, and snacks: $50/person

• Attend a peer group meeting and training session ONLY- breakfast and lunch: $25/person

• Attend a peer group meeting ONLY, no meals: Free
