SESSION 2: Advanced Care Planning: A New HEDIS Measure

SESSION OVERVIEW: The nurse’s role in assisting patients and families with Advanced Care Planning (ACP) is an important nursing responsibility, which can better prepare families to make decisions that honor patients’ preferences. Additionally, the documentation of an ACP is a new HEDIS measure. This session provides two components to meeting the new HEDIS measure. The first is an in-depth discussion of how to successfully conduct an ACP with a patient and their family, The second is the “nuts and bolts” of how to successfully utilize a nurse in your clinic to complete an ACP and receive payment for their services.
