SESSION 6: Community Health Worker Program Evaluation
WHEN: November 18, 2021 | 12:30pm - 1:30pm MST
WHERE: Virtual (link provided upon registration)
COST: $0 Members; $25 Non-Members
TARGET AUDIENCE: COO, Outreach & Enrollment, IT, Community Health Workers, Care Coordinators, and Quality Improvement staff
OVERVIEW: The lack of standardized measures to assess Community Health Workers (CHW) makes it difficult to aggregate data across programs and regions, impeding commitment to sustainable, long-term financing of CHW programs. The CHW Common Indicators Project’s purpose is to identify measures to understand the unique contributions of CHWs to successful program outcomes and their added value to the health care and human services systems. During this session, members of the CHW Common Indicators Project will present the measures developed by their organization, and their methodology with choosing these CHW program-specific measures to allow for reportable data across regions.
