TARGET AUDIENCE: Operations, Quality Improvement, and Health Center Boards

OVERVIEW: This learning collaborative will include information on accurately taking blood pressure, hypertension management and the role of a pharmacist, the correlation of hypertension and diabetes, self-monitoring blood pressure (SMBP) machines, automated blood pressure machines, and other related topics. At the end of the learning collaborative, participating health centers will receive a hypertension management toolkit to help with the implementation of SMBP devices.

AUCH partnered with the American Heart Association to provide AUCH's health center participants 10 SMBP devices, upon registration. Registered AUCH health center participants who attend six sessions of the learning collaborative will receive an additional 10-15 SMBP devices.
