SESSION OVERVIEW: 

Care pathways involve creating standardized clinical & administrative workflows that staff engage in to assist a consumer with a social determinant, physical, and/or behavioral health need. A care pathway operationalizes care management components into replicable, measurable workflow steps. Team-based care is a critical component of care pathways. The objectives of this one-hour webinar include:

• Define care pathways and their key components
• Identify roles of care team members in operationalizing care pathways
• Apply the care pathway development steps to your provider setting
