CHAMPS/RMPHTC Community of Practice (COP) Webinar

What: CHAMPS/RMPHTC COMMUNITY OF PRACTICE (COP): ADVANCED POPULATION HEALTH PRACTICES FOR COLORECTAL CANCER SCREENING  

Advanced Population Health Practices for Colorectal Cancer Screening COP will allow health center quality improvement (QI) staff to think beyond the basics and meet with peers to discuss ongoing and emerging needs in QI.  This series will focus on colorectal cancer screening as the metric of discussion, but the strategies and conversation will likely apply to quality improvement programs more broadly. The first session will begin with an open discussion among participants.  “What does population health mean at your health center?  How are you implementing population health practices to further your practice transformation efforts?” Come prepared to share your successes and hear from others in the region who may have overcome the struggles you are currently working through.  Conversation is open-ended, and you are encouraged to ask your own questions of your peers. For example, you might like to know how to prepare yourself for joining an Accountable Care Organization (ACO), or how to strengthen your compliance plan ahead of an On-Site Visit (OSV), etc. Subsequent sessions may then include invited panelists who can address questions raised by the community members.

Dates/Time: Advanced Population Health Practices for Colorectal Cancer Screening COP sessions will occur on the first Thursday of each month at 1:00-2:00 PM MT.

Audience: Quality Improvement and/or Clinical Operations/Practice Management Staff at Community Health Centers in Region VIII (CO, MT, ND, SD, UT, WY).

Registration: Please register on the RMPHTC webpage here. Participants are encouraged to attend the full series, but this is not required.  New members can register and join the COP at any time during the series. Registration will remain open until June 6, 2018.
