Evidence-Based Messaging about Colorectal Cancer Screening

CHAMPS and Rocky Mountain Public Health Training Center (RMPHTC) are pleased to announce that registration is now open for the NEW Care Coordination Strategies to Improve Rates of Colorectal Cancer Screening Project ECHO Learning Community.

In this series, health center care coordination and patient navigation professionals will develop key skills and strategies to improve patient outcomes related to colorectal cancer screening rates in support of the 80 by 18 goal set by the National Colorectal Cancer Roundtable (NCCRT).

Audience: The Care Coordination Strategies for Improving Rates of Colorectal Cancer Screening ECHO series is intended for those coordinating the care of patients at Region VIII Community Health Centers (CHCs).  This may include care coordinators, patient navigators, case managers, or similar roles.  Participating health centers are not required to have a care coordination program specific to colorectal cancer screening before attending this series.

JANUARY 23, 2018 – FEBRUARY 20, 2018

Dates: Care Coordination Strategies to Improve Rates of Colorectal Cancer Screening ECHO Learning Community will meet each Tuesday for five weeks beginning January 23, 2018 (1/23, 1/30, 2/6, 2/13, 2/20).

Time: 11:30 AM-12:30 PM MT (12:30 PM-1:30 PM CT)

  • January 30:  Evidence-Based Messaging about Colorectal Cancer Screening
  • February 6:  Patient Motivation
  • February 13:  Prioritizing Patients for Care Coordination
  • February 20:  Sustaining a Care Coordination Program
