Braiding Funding Streams for Healthcare and Supportive Housing Collaborations for Frequent Users

Limited resources and mounting pressure to reduce costs while also improving health outcomes have required supportive housing providers and health centers to identify innovative ways to stretch resources to better meet the needs of high-cost healthcare users in supportive housing.  This has meant using existing funding streams more creatively and identifying new opportunities to better serve this “frequent user” population.   This webinar examines traditional ways supportive housing programs and health centers are financed, and how they can partner to capitalize on emerging and innovative funding streams.  We will hear from a local program that has successfully “braided” funding sources together to achieve improved outcomes for some of their most vulnerable clients.
