Purpose

To advance the knowledge and skills needed for patients, families, caregivers and administrators to apply the science of integrated team based care to improving overall health outcomes for our community.

Objectives

Upon completion of this activity, participants should be able to

  • ​Identify strategies to empower patients and families as leaders in achieving their health goals
  • Develop and foster qualities needed for leaders to implement a continuum of integrated team based care
  • Review team processes and requirements to performing zero harm as related to suicide preventiono
  • Examine the integration of team coaching to improve outcomes for patients and families with high-risk needs
  • Construct practical strategies for successfully partnering with the primary care and community teams for managing opioid use, anxiety and traumatic stress, chronic medical conditions and maternal mental health​

CME Credits Available 

CME credits of 5.50 accredited through ACCME, ANCC, AAFP, NASW, UPA.  

Physician Registration Fee of $85.00 or Non-Physician Registration Fee of $35.00 can be waived for members of AUCH. Please contact Scott Hammer for more information and to have registration fees waived.
