The Introduction to Care Coordination curriculum consists of two days of intensive training to benefit medical assistants, community health workers, case managers, and others who are in contact with the patient and in team-based care.

Cost:

$250 each for two or more staff (SELECT GROUP REGISTRATION OPTION) per member organization
$350 each for single staff per member organization
$450 each non-AUCH member organization staff
