Fourth Street strives to provide whole-person health through an integrated medical behavioral health clinical model. Fourth Street’s behavioral health team consists of APRNs, LCSWs, and a medical assistant. Behavioral health staff work in collaboration with primary care providers and to deliver comprehensive care for homeless individuals. APRNs are responsible for providing group and individual therapy and providing medication management for behavioral health patients. Primary responsibilities include mental health and substance abuse screening, assessment, and diagnosis in accordance with the DSM-IV.

Core Responsibilities
• Conduct mental health and substance abuse screenings, crisis intervention, and brief psychiatric assessments to provide diagnostic clarification for triage, and brief behaviorally oriented interventions.
• Create appropriate treatment plans to include medications, psychotherapy, and appropriate diagnostic and laboratory interventions.
• Prescribe psychotropic medications.
• Perform individual psychotherapy
• Work in accordance with set protocols and procedures set forth by clinic management
• Maintain accurate and up to date documentation of all clinical encounters in the clinic’s electronic health record.
• Provide behavioral health consultation to, and work in conjunction with medical providers and their patients to meet the patient’s medical

• Must be licensed as a Psychiatric Advance Practice Nurse in the state of Utah, and able to practice without supervision.
• Prior experience with homeless population is preferred, but not required.
• Strong knowledge and experience working with individuals with co-occurring disorders (mental health and substance abuse) who are also service resistant.
• High degree of interpersonal competence with both staff and patients, and a thorough knowledge of evidence-based care.
• Spanish speaking is preferred, but not required.
