Job Description

Duties & Responsibilities
The following information is intended to be representative of the essential functions performed by incumbents in this position and is not all-inclusive.

  • Serve as a member of an integrated team and work to ensure patients have access to whole person care, including medical, pharmaceutical, and dental services.
  • Provide individual and group therapy and counseling for patients with mental health and substance abuse disorders.
  • Perform ASAM clinical assessments to inform treatment planning, goal setting, and referrals to outside entities for higher levels of care.
  • Assist clinic staff and work with clients that present in crisis by providing crisis management, assessing safety, and creating safety plans. 
  • Assist in resolving identified patient problems and needs by utilizing multidisciplinary team strategies.
  • Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
  • Provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders.
  • Provide targeted and accurate diagnostic clarification and consultation consistent with the DSM-5 to support primary care and specialty behavioral health teams. 
  • Assures quality of care by adhering to evidence-based practice, therapeutic standards, and standards of care set by state board of licensing, mental health professional practice act, social worker licensing act, and other governing agency regulations.
  • Supervision for both students and other clinical staff as needed.
  • Maintain accurate and up to date documentation of all clinical encounters in the clinic’s electronic health record.
  • Maintain patient confidence and protect operations by keeping information confidential.
  • Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications, establishing personal networks, participating in professional societies. 

Necessary skills/attributes

  • Competency in multiple psychotherapy modalities and evidence-based theoretical frameworks including but not limited to: harm reduction, trauma-informed care (TIC),  cognitive behavioral therapy (CBT), seeking safety, acceptance and commitment therapy  (ACT), motivational interviewing, and medication-assisted treatment (MAT).
  • Ethical and professional behavior.
  • Demonstrate excellent communication skills and be capable of working with diverse populations.
  • Interact and collaborate with professionals from many different social services and government agencies.
  • Demonstrate emotional resilience and stability.
  • Ability to work quickly when needed and be flexible with scheduling must be able to remain calm intense and/or chaotic situations.

Supervisory Relationships
This position reports to the Behavioral Health Clinical Director.


  • Graduate of an accredited master’s program in Social Work, or equivalent degree, and l licensed in the State of Utah.
  • BLS Certified within 90 days of hire.
  • Association of Social Work Board (ASWB) Clinical Level Exam, if applicable.
  • 4,000 hours of clinical social work and mental health therapy training, if applicable.
  • Bilingual English/Spanish preferred.
  • Must be able to pass a background check.


$48,000- $55,000
