Job Description

Works in conjunction with the Community Health Centers and the Patient Outreach and Support Team to help improve the health status, health knowledge, attitudes and practices of patients by promoting, supporting and assisting with follow-up calls, health assessments, home assessments, vital sign measurements, diabetes blood testing, non-emergency medical transportation, prescription delivery, health education and prevention. Frequently travels to assigned five-county service areas. Works with the elders, children, and others as necessary in the five-county service area. Supports the Project Coordinator for the Special Diabetes Program for Indians (SDPI) grant to assure all deliverables are met in a timely manner and costs are aligned with the scope of the project

General Requirements

• Must be insurable through the Tribe’s vehicle insurance.

• Must pass a criminal and fingerprint background check according to P.L. 101-630 standards.

• Must pass a pre-employment alcohol/drug screening.

• Must pass a physical examination prior to or contemporaneous with hiring.

Education and Experience:

• High School Diploma, equivalent GED or higher required.

• Two years’ experience in the health care field preferred.

• Work Experience with Native Americans preferred.

• Work experience and/or education in diabetes prevention, nutrition or fitness education preferred.

License & Certification

• Valid Utah Driver’s License and maintain thereafter.

• Basic Life Support (BLS) Certificate required and maintain throughout employment.

• Community Health Representative (CHR) Basic Training required or obtain within one (1) year of hire and maintain thereafter.

• Certified Nursing Assistant (CNA) preferred.

• Certified Lifestyle Coach preferred.
