Position Location: Utah, Summit, and Davis Counties, Utah

Position Description:

CHWs are trusted, knowledgeable frontline health personnel who typically come from the communities they serve. CHWs bridge cultural and linguistic barriers, expand access to coverage and care, and improve health outcomes.

AUCH is hiring CHWs to work in local communities as part of COVID-19 response efforts. CHWs will work primarily with underserved communities, particularly racial/ethnic minority communities, throughout Utah.

CHW responsibilities include, but may not be limited to, the following:

  • Establishing trusting relationships with patients and their families
  • Providing education about COVID-19, including testing and vaccination
  • Helping people who are exposed to or infected with COVID-19 to understand what is required for quarantine and isolation
  • Screening patients for Social Determinant of Health (SDOH) needs
  • Supporting contact tracing efforts as needed
  • Helping patients to establish a medical home
  • Documenting patient activities and outcomes in appropriate databases (documenting across several platforms is a large and time consuming responsibility of this role)
  • Participating in community and professional events as requested
  • Attending and participating in team meetings as assigned to discuss progress, successes, and challenges
  • Training new CHWs, including shadowing, and creating training and presentation materials
  • Other duties as assigned
  • You will receive training in how to best protect yourself from COVID-19, and ongoing support and training to allow you to work with patients and their families as safely as possible should you be required to report to any community-based sites. You will have access to the personal protective equipment (PPE) needed to do your work as safely as possible.

Organizational Background/Mission:

For over 30 years, the Association for Utah Community Health (AUCH) has represented Utah's Health Centers and their patients. As the Primary Care Association in the state, we provide over 12,000 hours of training and technical assistance to 13 Health Centers and 5 affiliate members each year. Our members include Federal Health Center awardees who provide comprehensive, high-quality primary and preventive health care services to all individuals, with or without insurance, regardless of ability to pay. Health Centers are community-based, patient-directed organizations that eliminate geographical and financial barriers and serve populations with limited access to care. In support of our members and the people they serve, AUCH helps reduce barriers to health care through health promotion, community engagement and development, education, and policy analysis.

Salary Range: DOE

Qualifications and Requirements: The following requirements must be met to be considered for the program:

  • High school diploma or GED
  • Written and oral fluency in English and Spanish required, other languages desired
  • Willingness to be present on-site at designated COVID-19 testing locations, if requested
  • Reliable transportation
  • Ability to work remotely and access to a reliable internet connection
  • Basic computer skills
  • Familiarity with documenting activities and entering data
  • Ability to develop and maintain positive relationships with patients and families
  • Understanding of, and sensitivity to, targeted communities
  • Good communication skills such as listening, speaking, and writing
  • Ability and willingness to provide emotional support, encouragement, and motivation to patients
  • Ability to work independently and as part of a group
  • Willingness to take initiative and be pro-active in daily work
  • Experience working within community partnerships preferred
  • Experience working as a Community Health Worker, or in a similar capacity, for at least one year is preferred
