Development Specialist”. |

This
position has specific responsibility for coordinating
special fund raising events, National Health Center Week
activities, individual donor annual giving campaigns and
assisting in the development and implementation of a
marketing plan to educate the public about the work and
mission of the Community Health Centers, Inc. The
“Resource Development Specialist” is responsible for
ensuring that events meet financial targets, stay within
projected budget and achieve overall goals as indicated
by event plans. This individual will help in building
community relationships and enhancing the visibility of
CHC within the community.
This position needs an individual with strong team
orientation, good internal and external customer service
skills, attention to detail, creative programming,
project planning abilities, and excellent organizational
abilities. Candidates must possess the flexibility to
handle multiple tasks and must be self-motivated. The
applicant must be proficient with Microsoft Access,
Excel, Word and internet-based research. The ability to
work in a highly collaborative, fast-paced environment,
necessitating qualities such as cultural awareness,
integrity and humor are also required. Strong verbal and
written communication skills are essential. A Bachelors
Degree in Business Marketing, Finance, Healthcare
Management or related field is helpful.
Hours and Compensation: Part-time hourly – compensation
dependent on experience.

CHC has an immediate opening for a
FT Physician
Assistant or Family Nurse Practitioner at our Stephen D.
Ratcliffe Clinic.
This professional will be responsible
for delivering primary care in an outpatient clinic
setting based on developed clinical protocols.
Completion of patient records, billing sheets, and leave
requests in accordance with the policies of CHC. Attend
provider meetings and bimonthly management meetings, and
participate as requested in Quality Assurance
activities. Duties also include on-call telephone
coverage after clinic hours in addition to occasional
weekends. Requirements include: graduation from an
accredited program, nationally certified within one year
of completion of program, maintaining national
recertification every six years and licensed in the
state of Utah in accordance with the Physician Assistant
Practice Act. Previous experience desired.

Outreach
Worker/Case Manager position
Southwest Utah Community Health Center, a non profit
primary health care agency, is seeking an independent,
highly organized individual to serve as Outreach
Worker/Case Manager. Responsibilities include working
with medical and mental health staff to support and
facilitate communication, services, and follow up care
for prenatal and chronic care patients, coordinating
well checks and immunizations with parents of children
under 2 years of age, assisting patients with keeping
appointments including referrals to specialists.
Excellent interpersonal skills, effective written and
verbal communication and detail oriented individual
needed. Bachelor’s degree or high school degree and
three years experience in a health related field.
Spanish and English speaking fluency required. This is a
full time, temporary position that will last one year.
It includes health insurance, sick and vacation leave,
